VI. PROCEDURES FOR FILING INSURANCE COVERED CLAIMS; REPORTING OF FIRE, SMOKE AND EXTENDED COVERAGE CLAIMS

Fire and Smoke

  1. Person discovering the fire or smoke is requested to alert individuals occupying the building to evacuate the premises by activating the fire alarm system.
  2. Person in charge of building is responsible for notifying the Greensboro Fire Department by calling 911 and the University Police Department at 334-7675.
  3. Person in charge of building is responsible for supervising the evacuation of building.
  4. Students, Faculty and Staff members are not to return to the building until a fire department official declares the area safe.
  5. The ARISA Officer is responsibile for notifying State Department of Insurance of any incidents involving covered claims.

All Risk Insurance Claims

Stolen or lost items which are covered under an all risk policy should be reported to the ARISA office. When reporting stolen or lost items, you must include descriptions, and serial numbers.

Reporting of Vehicle Accidents

1. Motor Fleet Management Leased Vehicles:

a. Report accident via telephone to Motor Fleet Management (MFM) at (919) 733-4043

b. Report accident to Traveler's Insurance Company at 1-800-762-3804

c. Complete Form FM-16 and submit to Motor Fleet Management with a copy to the University Vehicle Coordinator (Physical Plant).

2. University Owned Vehicles

a Report accident to Travelers Insurance Company at 1-800-762-3804.

b. Report accident to the department having assigned vehicle and ARISA Office.

All accidents involving injury or major property damage to the public should be reported promptly in the following manner:

To report accidents occurring in the Eastern counties listed, call collect the Travelers Claim Office in Richmond, VA (804-358-3856). These counties are:

Bertie, Currituck, Hertford, Pasquotank, Camden, Dare, Hydes, Perquimans, Chowan, Gates, Martin, Washington

To report accidents occurring in all other North Carolina counties, call collect the Travelers Claim Office in Charlotte, NC (1-800-762-3804). During non-business hours, on week-ends and holidays, when there is a serious injury or death, report the accident immediately by dialing direct (1-800-243-3840).

When an accident is reported, be prepared to answer as many of the following questions are applicable:

  1. What is your name, title and who is your employer?
  2. What is the code number for this vehicle? answer: Motor Fleet -6070; University Owned - 9260
  3. Business address of your employer.
  4. Location and description of accident.
  5. Description of state vehicle and driver's name.
  6. Description of other vehicles involved in accident and driver's name.
  7. Names of all persons injured in accident.
  8. Was accident investigated by police? Name of investigating officer, if known.
  9. Were there any other know witnesses?

Collision Insurance Coverage Reporting

Departments which have purchased collision insurance coverage for vehicles assigned are requested to report any accident to the ARISA Office at 334-7032.
