Procedures For Completing the Confidential Security Agreement

Please read the "Confidentiality Security Agreement" form before filling out the form. Pay special attention to item number 7 on the form. In signing the form, one is agreeing that the use of the sign-on and password issued is for their exclusive use. This sign-on account and password should not be shared with any other person.

Following is the format of the Confidentiality Security Agreement form. Please enter you name, social security number, department, and phone number. Sign and date the form and have your supervisor sign and date the form. Forward the completed document to Systems Development Office located in the room 220 of the Dowdy Building.
